The platform reads patient information from your EMR, then automatically submits completed documentation back after each visit. How It Works

Nestmed retrieves patient demographics, diagnoses, care plans, and other relevant clinical information from your EMR. When referral documents arrive in the EMR, Nestmed can retrieve and process them automatically, preparing summaries for the care team. After a visit, the completed documentation flows back to your EMR in the appropriate format. Visit notes, OASIS assessments, orders, and other documentation appear in the patient’s chart exactly where your team expects to find them. The synchronization happens in near real-time, ensuring that office staff and other clinicians always have access to the latest information.

Data Flow

The synchronization is intelligent and selective. Not every piece of data needs to flow between systems. During setup, you configure exactly what information should sync and how it should map between Nestmed and your EMR. This ensures that your existing workflows and data structures are preserved while adding Nestmed’s capabilities. Conflict resolution is handled gracefully, with clear rules about which system takes precedence for different types of data. Audit trails track every synchronization event, providing complete visibility into the data flow between systems.
